Support Coordination Support Plan

Support Coordination is a capacity building support intended to assist you to build the skills you need to understand, implement and use your NDIS plan. As a support coordinator, we work with you to connect with informal, community and NDIS funded supports working to increase capacity to maintain relationships, manage service delivery tasks and live more independently in the community.

Support Coordinators do not

  • Determine whether or not supports or services which have been purchased are 'reasonable and necessary'

  • Decide what supports you can access

  • Sign documentation on your behalf

  • Make changes to your NDIS funding

Support Coordinators do

  • Facilitate choice and control

  • Appreciate the subjectivity of what supports a you can purchase to work towards your goals

  • Build capacity for you to answer the question 'What can I spend my NDIS funds on?'

  • Assist you to navigate your NDIS plan and connect you with supports

Breakdown

Implementing my NDIS Plan

Supporting me to understand my current plan and mapping supports for the plan. This includes, but is not limited to:  Implementation meeting, co-designing your support plan, Review your NDIS goals, Research, Engagement with Support Providers, Budget, support to implement crisis plan if required, support to implement emergency and disaster management plan and NDIS reporting requirements.

Plan Monitoring

This is includes, but is not limited to:  regular check in's, review of budget, adjustments to supports as required, NDIS reporting requirements

Preparing for my next plan 

This includes, but is not limited to:  Reviewing existing plan with you to determine what worked and what needs to change, coordinating reports from providers to support funding in your new plan, NDIS reporting requirements, budget, Pre-Plan Review meeting with you, Attending Plan Review Meeting with you.
